95% reduction
95% reduction is available to a guardian or guide accompanying:
1. persons with war and military disabilities classified in:
- group I of disability or declared totally incapable of working and unable to lead an independent life, even if unconnected with warfare or military service,
- group II or III of disability, or completely or partially incapable of working and, at the same time, classified in group I disability (or declared completely incapable of working and living independently), due to a general health condition, with a war-military disability book.
2. Veterans with war or military disability classified in group I of disability or declared completely unable to work and unable to lead an independent life, with a veteran's card.
3. Persons incapable of independent living, with the exception of blind persons (a person with a severe disability or disability of group I should also be regarded as a person incapable of independent living.
Eligibility for the reduction is evidenced by one of the following documents:
- an extract from the decision of the medical committee for invalidity and employment, stating that the person is classified in group I of disability,
- an extract from the decision of: the military medical commission, the medical commission subordinate to the Ministry of Internal Affairs and Administration or the Head of the Internal Security Agency or the Head of the Foreign Intelligence Agency, stating that the person is classified in group I of disability,
- a decision of an examining doctor of the Social Insurance Institution (ZUS) or an extract from such decision of an examining doctor of the Social Insurance Institution (ZUS) stating complete inability to work and inability to lead an independent life, or inability to lead an independent life,
- decision of the medical commission of the Social Insurance Institution (ZUS) stating complete inability to work or complete inability to work and inability to lead an independent life, or inability to lead an independent life,
- a certificate from the Social Insurance Institution (ZUS) stating that the person has been classified in group I of disability by a court judgement or has been declared incapable of independent living,
- an extract from the decision of an expert doctor or the medical commission of the Agricultural Social Insurance Fund (KRUS) stating complete inability to work,
- an extract from the decision of the Agricultural Social Insurance Fund (KRUS) medical commission stating incapacity of independent living,
- a certificate from the Agricultural Social Insurance Fund (KRUS) stating that the person has been classified in group I of disability by a court judgement or has been declared incapable of independent living,
- an identity card of a retired police employee issued by a competent retirement authority subordinate to the Minister of the Internal Affairs or to the or the Minister of Justice, respectively, with an entry stating that the employee has been classified in group I of disability,
- an identity card of a retired police employee issued by a competent retirement and disability pension authority subordinate to the Minister of the Internal Affairs and Administration, with an entry stating that the employee has been classified in group I of disability, or that they have been declared incapable of independent living,
- an identity card of a retired police employee issued by a competent retirement and disability pension authority subordinate to the Minister of National Defence, with an entry stating that the employee has been classified in group I of disability, or that they have been declared incapable of independent living,
- an identity card of a retired army employee issued by a competent retirement authority subordinate to the Minister of National Defence, with an entry stating that the employee has been classified in group I of disability,
- an identity card of a disabled person certifying a significant degree of disability, issued by an authorised body.
4. Blind persons, if they are classified as unable to lead an independent life (a person with a significant degree of disability or with group I disability is also classified as unable to lead an independent life) due to a condition of sight, and blind persons, if they are not classified as unable to lead an independent life, that is, persons classified as totally incapacitated, persons with a moderate degree of disability and persons with group II disability, due to a condition of sight.
Eligibility for reduction shall be certified by one of the documents listed in item 3, confirming inability to lead an independent life or a group I disability, or a significant degree of disability due to a visual impairment - or a certificate of significant disability due to a visual impairment issued by an authorised body.
5. Civilian blind victims of warfare declared as:
- persons incapable of independent living (a person with a severe disability or disability of group I should also be regarded as a person incapable of independent living),
- persons completely incapable of working, with a civilian blind victim of war card, with the appropriate entry (stating incapacity for independent existence or total incapacity for work), issued by the pension authority.
A guardian or guide means one person of legal age or, in case of a guide for a blind person, a person who is at least 13 years old, or a guide dog.
Documents without a photograph shall be presented along with an identity card or any other document allowing to confirm the identity of the eligible person.
